Advocate Health in Charlotte, NC seeks an RN/NP leader to oversee clinical triage, disease management, discharge call management, patient navigation, and virtual care coordination in an outpatient setting. You will support the development and implementation of a patient navigation center and ensure CI information systems integration.

Required are an RN license and MSN; if the undergraduate degree is a BSN, a Master’s degree in a health-related field or MBA is acceptable.

• MSN required; if BSN only, Master’s in health-related field or MBA acceptable
• Professional leadership certification required within 1 year of eligibility

• Oversees clinical triage services, disease management, discharge call management, patient navigation, and virtual care coordination.
• Drives development and implementation of a patient navigation center.
• Ensures data collection and reporting supports clinical operations and CI integration.
• Manages department budget and guidelines; reports on clinical services.
• Establishes governance and engagement structures to sustain system-wide strategies.
