Job summary

Qualifications

• Current, unencumbered RN license in the state of practice.
• Minimum of three years of nursing experience, with at least one year in skilled nursing.
• Demonstrated wound assessment and treatment experience.
• Working knowledge of the federal requirements of participation for long term care facilities and of the survey process.
• Proficiency with an electronic health record; experience with PointClickCare strongly preferred.
• Ability to communicate clearly and professionally with residents, families, physicians, staff, surveyors, and payers.

Responsibilities

• Lead wound care program assessment, treatment, and documentation.
• Coordinate wound consults, vascular studies, debridement, NP therapy, and referrals; ensure implementation of recommendations.
• Design, supervise, and evaluate the formal restorative nursing program with measurable goals and outcomes.
• Ensure program readiness for surveys and maintain quality measures, care planning, and documentation.
• Supervise Restorative Aides, train staff, and coordinate with Therapy to prevent service duplication.
