Genesis HealthCare in Carlisle, PA, is seeking a Director of Social Services to lead the Social Services department with a focus on resident rights, dignity, and recovery in a long-term care setting. You will guide transitions, collaborate with an interdisciplinary team, and champion psychosocial care at the highest standards.

You will oversee budgets, staffing, and quality initiatives, mentor social workers, and ensure compliance with NASW Code of Ethics and state regulations while driving

Formación

• Master’s degree in Social Work from an accredited school of Social Work required.
• Must possess certifications/licensures as required by State of employment to practice in long term care.
• Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Abuse preferred.
• At least 3-5 years of supervised Social Service experience in a health care setting preferred.
• Management/administrative/supervisory experience preferred.

Responsabilidades

• Clinical Excellence: Conduct psychosocial assessments, develop interdisciplinary care plans, and provide therapeutic interventions for long-term care residents.
• Strategic Administration: Manage departmental budgets, staffing, and quality improvement initiatives; be the Social Services voice in meetings.
• Discharge Mastery: Coordinate transitions back to the community with linkages to behavioral health and hospice.
• Empowerment through Education: Train staff on resident rights and educate families on healthcare decision-making.
