You will ensure comprehensive psychosocial assessments, protect patient rights, and collaborate with the interdisciplinary team to optimize residents’ quality of life.

Qualifications

• Bachelor's degree in social work or human services required.
• Must possess any certifications/licensures as required by state of employment to practice in long-term care.

Responsabilités

• Foster an environment within the nursing center that enables patients/residents to maximize well-being and quality of life.
• Plan, develop, organize, evaluate, and direct the Social Services department in accordance with NASW Code of Ethics and laws.
• supervise staff of up to 2 employees.
• Collaborate with social services staff and administration to promote patient rights, independence and dignity.
• Educate patients and families regarding rights, decision making and community resources.
• Provide therapeutic interventions to aid transition and adjustment to a nursing center.
