American Medical Administrators MGMT Company is seeking a Social Services Director to oversee social services for residents and their families. The role emphasizes empathy, coordination with healthcare professionals, and compassionate support for residents and their relatives.

The director will conduct admissions assessments, follow up on concerns, arrange resources, and guide the team in loss support and documentation for benefits such as Medicaid and SSI.

Qualifications

• Minimum of a Bachelor’s degree in social work; OR
• Minimum of a Bachelor’s degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling or special education; AND
• One year in a healthcare setting where the individual is working as a supervised social worker who works directly with individuals

Responsibilities

• Responsible for the provision of Social Services to residents of the facility and their families under the direction of the social services director.
• Meet new residents upon admission and complete an Initial Psychosocial Assessment; visit residents to follow up on concerns.
• Arrange to purchase needed items for residents and ensure residents have adequate clothing items.
• Schedule visits to dental, eye, podiatrist and audiologist.
• Provide support in times of loss to residents and family.
• Assist residents in gathering supporting documents for Medicaid, Medicare, SSI, etc.
• Any other duties directed by the Social Services Director.
