Job summary

Spyglass Healthcare is seeking a Social Services Director to lead the Social Services Department in a skilled nursing and behavioral health setting. You will evaluate and organize services to meet residents’ emotional and social needs on an individual basis, while ensuring compliance with regulations.

Responsibilities include developing care plans, coordinating transitions, acting as mediator for concerns, and maintaining documentation with a focus on resident rights and quality of care.

Qualifications

• Bachelor’s degree in Sociology or Psychology (or equivalent).
• Strong communication skills (verbal & written).
• Experience in developing social care plans, patient assessments and discharge planning.
• Minimum 2 years’ experience in hospital or skilled nursing setting.

Responsibilities

• Develops a comprehensive social history and assessment for care planning.
• Assists residents and caregivers with transitions, orientation, and rights education.
• Acts as mediator to resolve care/treatment, admission, and discharge concerns.
• Participates in development of individual care plans for Social Services goals.
• Coordinates care services and attends multidisciplinary meetings.
• Ensures Advanced Directives are completed and filed.
• Develops discharge plans and coordinates transfers for continuity of care.
• Maintains relationships with residents, families, physicians, and staff.
