Job summary

Capitol Rehabilitation and Healthcare Center is seeking a Director of Social Services to lead psychosocial programs and advocate for residents and families. This full-time role reports to leadership and requires a clinical, compassionate approach to care coordination in a Sub-Acute and Long-Term Care setting.

The position emphasizes compliance with federal and state regulations, interdisciplinary collaboration, and effective communication with residents, families, staff, and external agencies.

Qualifications

• Bachelor’s degree required; Master’s degree strongly preferred.
• Current LSW/CSW licensure required.
• Experience as a social services director in a healthcare setting.
• Experience in a nursing home or rehabilitation center preferred.
• Knowledge of regulations governing nursing care facilities.
• Excellent communication with residents, families, staff, and external agencies.

Responsibilities

• Ensure services meet regulatory standards and guidelines.
• Collaborate with interdisciplinary teams for resident-centered care planning.
• Provide counseling, support, and advocacy during admission, transitions, and end-of-life planning.

Responsibilities for Director of Social Services:

• Provide leadership and oversight of social services programs to support the emotional, psychosocial, and practical needs of residents and their families.

• Ensure services are delivered in accordance with all applicable federal, state, and facility regulations, guidelines, and standards of care.

• Collaborate with interdisciplinary teams to support resident-centered care planning and ensure holistic support for residents’ well-being.

• Provide compassionate counseling, support, and advocacy for residents and families during admission, care transitions, and end-of-life planning.

• Serve as a key liaison between residents, families, staff, and external agencies to ensure effective communication and coordinated care.

• Support and guide staff in addressing resident concerns, behavioral challenges, and psychosocial needs with empathy and professionalism.

• Assist with discharge planning, resource coordination, and community referrals to support smooth transitions of care.

• Maintain accurate documentation and records in compliance with regulatory requirements and facility policies.

Qualifications for Director of Social Services:

• Bachelor’s degree required; Master’s degree strongly preferred.

• Current LSW/CSW licensure required.

• Experience as a social services director required

• Experience as a social worker in a healthcare setting is required.

• Experience in a nursing home or rehabilitation center strongly preferred.

• Strong knowledge of regulations, standards, and guidelines governing nursing care facilities.

• Excellent communication skills with the ability to interact effectively and compassionately with residents, families, staff, and external agencies.

• Ability to balance strong advocacy with empathy, professionalism, and emotional support.
