Job summary

Westerly Rehabilitation and Healthcare Center in Rhode Island is seeking a Director of Social Services to lead psychosocial programs, advocate for residents, and guide families through care journeys with empathy and professionalism.

In this leadership role you will ensure regulatory compliance, coordinate with interdisciplinary teams, oversee discharge planning, and support transitions and end-of-life planning for residents and their loved ones.

Qualifications

• Bachelor’s degree in social work or related field is required.
• Current LSW/CSW licensure required.
• Experience as a social worker in a healthcare setting is required.
• Experience in a nursing home or rehabilitation center is preferred.
• Strong knowledge of regulations and guidelines governing long-term care facilities.
• Excellent communication skills with residents, families, staff and external agencies.
• Ability to balance advocacy with empathy, professionalism and emotional support.

Responsibilities

• Ensure services meet federal, state and facility regulations and standards.
• Collaborate with interdisciplinary teams to support resident-centered care planning.
• Provide counseling, support and advocacy during admission, transitions and end-of-life planning.

• Guide staff in addressing resident concerns and psychosocial needs with empathy.
• Assist with discharge planning, resource coordination and community referrals.
• Maintain accurate documentation in compliance with regulatory requirements.

Responsibilities for Director of Social Services:

• Provide leadership and oversight of social services programs to support the emotional, psychosocial, and practical needs of residents and their families.
• Ensure services are delivered in accordance with all applicable federal, state, and facility regulations, guidelines, and standards of care.
• Collaborate with interdisciplinary teams to support resident-centered care planning and ensure holistic support for residents’ well-being.
• Provide compassionate counseling, support, and advocacy for residents and families during admission, care transitions, and end-of-life planning.
• Serve as a key liaison between residents, families, staff, and external agencies to ensure effective communication and coordinated care.
• Support and guide staff in addressing resident concerns, behavioral challenges, and psychosocial needs with empathy and professionalism.
• Assist with discharge planning, resource coordination, and community referrals to support smooth transitions of care.
• Maintain accurate documentation and records in compliance with regulatory requirements and facility policies.

Qualifications for Director of Social Services:

• Bachelor’s degree required; Master’s degree strongly preferred.
• Current LSW/CSW licensure required.
• Experience as a social worker in a healthcare setting is required.
• Experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, standards, and guidelines governing nursing care facilities.
• Excellent communication skills with the ability to interact effectively and compassionately with residents, families, staff, and external agencies.
