Delmar Center is seeking an experienced Director of Social Services to lead our social services department in a skilled nursing facility. The role focuses on guiding psychosocial support for residents, coordinating programs, and ensuring regulatory compliance across the organization.

The successful candidate will oversee staff, develop individualized care plans, advocate for residents and families, and collaborate with interdisciplinary teams to maintain high quality care and rights protection.

Qualifications

• Bachelor's degree in social work required.
• Experience in a nursing home, long-term care, or healthcare setting required.
• Supervisory or leadership experience preferred.
• Knowledge of state and federal long-term care regulations.

Responsabilités

• Direct and oversee all social services programs and activities within the facility.
• Complete resident psychosocial assessments and develop individualized care plans.
• Provide counseling, support, and advocacy for residents and their families.
• Coordinate discharge planning and transitions of care.
• Ensure residents' rights are protected and maintained.
• Participate in interdisciplinary care plan meetings.
• Supervise and mentor social services staff.
• Maintain accurate documentation and ensure compliance with state and federal regulations.
• Serve as a liaison between residents, families, community agencies, and facility staff.
