Job summary

Heartland Nursing and Rehabilitation Center in Boynton Beach, FL, is seeking an experienced Director of Social Services. This full-time role leads the Social Services department, advocates for residents, and guides discharge planning to support dignified healthcare journeys.

The position requires a BSW or related field, at least two years in healthcare/LTC, and strong leadership and communication skills. Comprehensive benefits are provided.

Qualifications

• Bachelor’s degree in Social Work (BSW) or related human services field preferred.
• Minimum of two years of experience in a healthcare or long-term care setting.
• Strong knowledge of federal and state long-term care regulations.
• Excellent communication, leadership, organizational, and problem-solving skills.
• Ability to support residents and families through sensitive situations with empathy and professionalism.

Responsibilities

• Develop, implement, and maintain Social Services policies and procedures in accordance with applicable federal and state regulations.
• Lead, mentor, and support the Social Services team.
• Advocate for resident rights, dignity, choice, and quality of life.
• Assess residents’ psychosocial needs and collaborate with the interdisciplinary team to develop individualized care plans.
• Participate in care planning meetings and maintain communication with residents, families, and facility staff.
• Coordinate discharge planning, including community resources, referrals, and services needed for a successful transition.
• Maintain accurate, timely documentation and support survey readiness.
• Monitor departmental performance, identify opportunities for improvement, and prepare Quality Assurance reports.
• Serve as the facility’s Grievance Officer, ensuring concerns are documented, investigated, and addressed promptly and respectfully.

Heartland Nursing and Rehabilitation Center is seeking an experienced and compassionate Director of Social Services to lead our Social Services department and support the well‑being of our residents and their families.
