Job summary

Trilogy Health Services, LLC is seeking a Director of Social Services to lead social care plans and resident assessments at the New Albany Health Campus in Ohio. The role focuses on meeting medically related emotional and social needs through individualized care planning and coordination with families and external services.

The position requires a Bachelor's in Social Work or related field, with 3–5 years in senior care social services and licensure eligibility.

Qualifications

• Bachelor's or advanced degree in Social Work or a Human Services field, including but not limited to sociology, psychology, gerontology.
• Three to five years' experience in social services in senior residential care, healthcare, or long-term care.
• Must meet state licensure requirements and regulatory codes for licensure or certification.

Responsibilities

• Plan, develop and organize the social service programs of the Health Campus.
• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Develop a written plan of care for each resident identifying problems, needs and goals.
• Provide information on Medicare/Medicaid and other financial assistance programs.
• Coordinate outside support services to meet resident care needs at home or on campus.
• Assist in transitional care planning from hospital or facility to home or assisted living.

POSITION OVERVIEW

The Director of Social Services is primarily responsible for the implementation of social care plans and resident assessments to ensure that the medically related emotional and social needs of the residents are met and maintained on an individual basis.

Key Responsibilities

• Plan, develop and organize the social service programs of the Health Campus.
• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
• Provide information to residents/families on Medicare/Medicaid and other financial assistance programs available to the residents.
• Participate in IHCC allowed activities such as the development and oversight of a healthcare support plan that includes coordination of medical care and proactive care management of both chronic diseases and complex conditions such as recurring falls, depression and dementia. Assist in transitional care planning from hospital or nursing facility to home/assisted living.
• Coordinate outside support services for residents to meet their care needs at home or in the health campus.

Qualifications for Director of Social Services I:

Indiana locations

• Bachelor's or advanced degree in Social Work or a Human Services field, including but not limited to:
  • Sociology;
  • Special Education;
  • Rehabilitation Counseling;
  • Psychology;
  • Gerontology; and one (1) year of supervised social service experience under the supervision of a qualified social worker in a health care setting directly working with individuals.
• Three (3) to five (5) years' experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment

Michigan locations

• Registered Nurse (RN) or Nurse Practitioner (NP);
• Licensed bachelor or master degreed Social Worker;
• Licensed professional Counselor;
• Psychologist;
• Physician's Assistant (PA) or Physician
• Must meet State requirements and regulatory code(s) for licensure or certification
• Three (3) to five (5) years' experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment

KY, OH, WI locations

• High school diploma or equivalent;
• Bachelor Degree in a Human Services field is NOT required;
• 3-5 years experience working in long-term care, social services is required;
• Licensed practical nurse in Kentucky, Wisconsin, or Ohio

All Locations

• Must meet State requirements and regulatory code(s) for licensure or certification
• Three (3) to five (5) years' experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment
• Exemplary computer skills including knowledge of Microsoft Office Suite Products

Qualifications for Director of Social Services II:

• Bachelor's degree in social work
• LSW preferred
• Must meet State requirements and regulatory code(s) for licensure or certification
• Three (3) to five (5) years' experience working in the field of social services in a senior residential care, healthcare, senior living industry or long-term care environment

Compensation will be determined based on the relevant license or education held, as well as the candidate's years of experience
