Job summary

CareCore Health LLC is seeking an experienced Social Services Director for our Long Term Care Facility in the Cincinnati area. The ideal candidate will lead all social services activities, advocate for residents, and coordinate support services to enhance well-being and placement experiences.

Responsibilities include supervising social services staff, collaborating with the interdisciplinary team, and ensuring compliance with federal, state, and local regulations.

Qualifications

• Bachelor’s Degree in Social Work or related field
• Licensed Social Worker (LSW) in Ohio
• Minimum of 2 years in a long-term care or healthcare setting
• Strong knowledge of Medicare/Medicaid regulations and discharge planning
• Excellent communication, organizational, and documentation skills

Responsibilities

• Overseeing all social services functions within the facility
• Assisting residents and families with adjustment to placement
• Providing support for emotional, social, and psychological well-being
• Developing and updating individualized care plans
• Coordinating discharge planning and community resource referrals
• Ensuring compliance with federal, state, and local regulations
• Leading and supervising social services staff, if applicable
• Collaborating with interdisciplinary team members to ensure holistic care
