Centra Healthcare Solutions is seeking a Director of Rehabilitation (DOR) to join a growing team in a SNF/LTC setting. This is a direct hire/permanent position offering leadership over the rehab program and multidisciplinary collaboration.

The DOR will oversee clinical planning, program operations, and staff development across PT/OT/SLP disciplines, ensuring adherence to physician orders and organization goals. A strong leadership background in rehab is required.

Competenze

• Must have graduated from an accredited school.
• Must have previous DOR experience.
• Current state license as an OT/PT/SLP/COTA/PTA or in the process of licensure application.
• Current CPR certification.

Job Title

Director of Rehabilitation / Director of Rehab / DOR to join a growing team at a great SNF / LTC. This is a direct hire / 'perm' position.

Key Responsibilities

• Incorporate the vision, values, mission and critical goals of the organization into job performance.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
• Provide skilled therapy services / interventions in accordance with physician orders.
• Advises, consults and instructs members of the multi-disciplinary therapy staff in specific physical, occupational and speech therapy modalities and interventions.
• Responsible for the overall administration, clinical planning, development and operations of the rehab program.

Requirements

• Must have graduated from an accredited school.
• Must have previous DOR experience.
• Current state license as an Occupational Therapist / OT, Physical Therapist / PT, Speech Language Pathologist / SLP, Occupational Therapy Assistant / COTA or Physical Therapy Assistant / PTA or proactively in the process of the application process for current state licensure as an OT, PT, SLP, COTA or PTA.
• Current CPR certification.
