Job summary

Millbrae Healthcare Center is seeking a Director of Rehabilitation to lead the rehabilitation department for a skilled nursing setting. The role requires licensure in CA and prior leadership within rehab, with responsibilities spanning clinical oversight, team mentorship, and regulatory compliance.

You will collaborate with nursing and medical staff to deliver resident-centered care, oversee care plans, and drive PDPM processes while maintaining high standards of quality and performance.

Qualifications

• Active California license in Physical Therapy, Occupational Therapy, or Speech-Language Pathology, in good standing.
• Minimum of one year of experience as a Director of Rehabilitation in a skilled nursing facility is required.

Responsibilities

• Lead and manage the daily operations of the rehabilitation department.
• Supervise, mentor, and support Physical Therapists, Occupational Therapists, Speech-Language Pathologists, assistants, and therapy aides.
• Ensure compliance with federal, state, and facility regulations, including documentation standards.
• Collaborate with nursing, physicians, and interdisciplinary teams to coordinate resident care.
• Develop and oversee individualized care plans and support PDPM processes.
• Monitor productivity, clinical outcomes, and departmental performance metrics.
• Participate in audits, care conferences, and leadership meetings.

WhatWe’reLooking For

• Minimum of one year of experience as a Director of Rehabilitation in a skilled nursing facility isrequired
• Bachelor’s degree in Physical Therapy, Occupational Therapy, or Speech-Language Pathology (or higher)
• Prior leadership experience within a rehabilitation department preferred
• Strong knowledge of PDPM, Medicare, and skilled nursing facility regulations
• Experience with EMR systems and regulatory compliance
• Excellent leadership, communication, and organizational skills
• Ability to foster teamwork, professional growth, and clinical excellence
• Passion for serving the geriatric population
