Job summary

HOME-HEALTHMED SERVICES® LLC is seeking a Director of Nursing to lead clinical operations for our Hampton Roads home health services. You will supervise care plans, support caregivers, and ensure safe, high-quality care while contributing to growth through hospice staffing partnerships.

You will oversee care plan development, supervisory visits, caregiver training, infection control, and clinical documentation. A Virginia RN license and independence in practice are required.

Qualifications

• Current, active, unencumbered Virginia RN license (or multistate privilege to practice in Virginia), in good standing
• Able to clear a Virginia criminal background check with no barrier crimes
• Current TB screening and required health clearances
• 2+ years of home health, home care, or personal care experience preferred
• Dependable, organized, and comfortable working independently

Responsibilities

• Assess new clients and develop individualized written plans of care
• Conduct periodic supervisory visits and keep care plans current
• Be reachable by phone or secure message during service hours for clinical questions
• Plan and lead caregiver in-service training and competency evaluations
• Oversee infection control practices and clinical documentation
• Support intake and coordination for hospice-referred clients as we grow

About the Role

As our DON, you are the clinical leader responsible for supervising the personal care services we provide to clients across Hampton Roads. You'll oversee client care plans, support and evaluate our caregivers, and help ensure we deliver safe, high-quality, compliant care. You'll also play a key part in our growth as we build hospice staffing partnerships.
