Job summary

Savant Senior Living in Downey, CA seeks an experienced Nurse Leader to assist the DON in planning, directing clinical services for home health patients and to supervise RNs, LPNs, and aides.

The role requires California RN license, BSN preferred, 2 years nursing with 1 year in home health or supervision; travel to patient homes may be required; on-call duties evenings/weekends; EHR proficiency.

Qualifications

• Current and active RN license in California.
• BSN preferred.
• Minimum of 2 years nursing experience, with at least 1 year in home health or supervisory role.
• Knowledge of Medicare Conditions of Participation and OASIS documentation.
• Strong leadership, communication, and organizational skills.
• Ability to multitask and work in a fast-paced environment.
• Proficient in electronic health records (EHR) systems.

Responsibilities

• Assist the DON in planning, organizing, and directing all clinical services for home health patients.
• Supervise and support RNs, LPNs, and home health aides in the delivery of patient care.
• Participate in hiring, training, performance evaluations, and scheduling of clinical staff.
• Ensure compliance with regulatory requirements, accreditation standards, and agency policies.
• Conduct case conferences and clinical record reviews to ensure quality care and accurate documentation.
• Monitor patient outcomes and implement improvement plans as necessary.
• Participate in on-call rotation and provide clinical support during emergencies.
• Serve as a liaison between clinical staff, patients, families, physicians, and community partners.
• Help develop and implement staff education programs and competency assessments.
• Maintain accurate documentation and assist with audits, surveys, and quality assurance activities.
• Office and field-based role; may require occasional travel to patient homes.
