Job summary

Centennial Healthcare in Southwest Colorado seeks a Director of Nursing to lead the nursing department with a focus on resident-centered care and clinical excellence.

The DON will guide a multidisciplinary team, drive quality improvement, and ensure compliance with state and federal regulations while fostering a positive, accountable nursing culture and staff development.

Qualifications

  • Current RN license in Colorado or eligibility to obtain Colorado licensure.
  • Previous nursing leadership experience preferred.
  • Experience in skilled nursing or long-term care setting.
  • Strong knowledge of state and federal long-term care regulations.
  • Experience with survey preparation, quality improvement, and clinical compliance.
  • Strong leadership, communication, organization and problem-solving skills.
  • Commitment to resident-centered care and professional accountability.

Responsibilities

  • Lead, oversee, and support all nursing operations within the facility.
  • Provide leadership, mentorship, and guidance to nursing staff.
  • Promote high standards of resident-centered care and clinical excellence.
  • Monitor resident care practices and identify opportunities for improvement.
  • Ensure nursing services remain compliant with state and federal regulations.
  • Support survey readiness and ongoing quality improvement initiatives.
  • Collaborate with facility leadership and interdisciplinary teams to improve resident outcomes.

  • Support staffing efforts, employee retention, and nursing team development.
  • Participate in the hiring, onboarding, coaching, and development of nursing employees.
  • Monitor clinical documentation and ensure accuracy and compliance.

A well-established skilled nursing and rehabilitation community in Southwest Colorado is seeking an experienced and compassionate Director of Nursing (DON) to lead our nursing department.

What You'll Do

As the Director of Nursing, you will provide leadership and direction to the nursing department while working closely with facility leadership and the interdisciplinary team.

What We're Looking For
  • Current RN license in Colorado or eligibility to obtain Colorado licensure

  • Previous nursing leadership experience preferred

  • Director of Nursing, Assistant Director of Nursing, or similar nursing leadership experience in a skilled nursing or long-term care setting strongly preferred

  • Strong knowledge of state and federal long-term care regulations

  • Experience with survey preparation, quality improvement, and clinical compliance

  • Strong leadership and team-building abilities

  • Excellent communication, organization, and problem-solving skills

  • Ability to lead with professionalism, accountability, and compassion
