Role Description
This is a full-time on-site role for a Registered Nurse split (50/50) between Boston Adult Daycare dba Sarah Care and Sarah Care Adult Foster Care.
Qualifications
Primary Responsibilities:
Clinical Oversight
Provide clinical leadership and supervision for all nursing staff within the ADH and AFC programs.
Ensure compliance with all state and federal regulations (e.g., MassHealth, EOEA guidelines).
Conduct routine assessments and reassessments of participants’ medical needs and service plans.
Care Planning & Documentation
Oversee the development, implementation, and evaluation of individualized care plans.
Ensure accurate and timely documentation in accordance with Medicaid/MassHealth requirements.
Coordinate interdisciplinary team meetings and updates to care plans as needed.
Health & Safety Compliance
Monitor and ensure adherence to infection control protocols and health and safety regulations.
Conduct regular audits and reviews of clinical records, incident reports, and emergency procedures.
Lead emergency preparedness and response efforts for both settings.
Staff Training & Supervision
Recruit, train, and evaluate nursing and caregiving staff.
Provide ongoing education and competency assessments for licensed and unlicensed staff.
Promote a culture of clinical excellence, safety, and ethical care.
Participant & Family Communication
Serve as the clinical liaison for participant families and caregivers.
Support care transitions, such as hospital discharges or changes in health status.
Address concerns or complaints with professionalism and empathy.
Regulatory & Quality Assurance
Ensure programs meet all licensure and accreditation standards (e.g., DPH, Medicaid).
Lead quality improvement initiatives and prepare for audits or inspections.
Maintain and update nursing policies and procedures in line with best practices.
Coordination with Primary Care Providers & Vendors
Communicate regularly with participants' PCPs and specialists to support continuity of care.
Oversee the coordination of outside services such as physical therapy, home health, or DME.
Program Support & Development
Collaborate with program administrators to support the strategic goals of the ADH and AFC programs.
Participate in budget planning for clinical staffing and medical supplies.
Help develop clinical protocols and expand service capacity.
