Job summary

At Home Harmony in Richmond, VA seeks an experienced Director of Dementia Services to lead and expand the GUIDE dementia program. You will oversee operations, clinicians, care navigators, and partnerships while ensuring CMS GUIDe requirements and quality metrics are met.

The role emphasizes building a high-impact team, expanding dementia services, and improving outcomes for patients and caregivers in a growing home-based care model.

Qualifications

• Bachelor's degree in nursing, social work, healthcare administration, or related field.
• Experience working with individuals living with dementia or Alzheimer's and their caregivers.
• Leadership experience with teams, workflows, performance, and cross-functional collaboration.
• Experience with CMS, Medicare, value-based care, care management or other regulated programs.
• Excellent communication, organization, analytical and problem-solving skills.

Responsibilities

• Lead day-to-day operations and growth of the GUIDe dementia program.
• Ensure CMS GUIDe requirements, quality measures, documentation, and reporting.
• Develop interdisciplinary team of clinicians, care navigators, social workers, and other partners.
• Provide education and care coordination to patients and caregivers.
• Build relationships with community organizations and referral partners.
• Monitor performance, enrollment, utilization, caregiver outcomes, and financials.
• Recruit, train, and develop team members; collaborate with leadership.
