Job summary

ACMH INC in Manhattan, NY seeks a Director of Intake & Transition Services to coordinate agency intake and promote client transitions to independent living. You will report to the Senior Director for Residential Programs, supervise Transition Specialists, and liaise with hospitals, clinics, shelters, and housing providers. Schedule is Monday–Friday, 9:00am–5:00pm.

Requirements include an M.A. in psychology (or equivalent) or a BA with substantial field experience; salary ranges are competitive

Qualifications

• Experience coordinating intake and transition processes.
• Strong liaison with hospitals, clinics, and housing providers.
• Supervisory experience of transition specialists.

Responsibilities

• Coordinate agency intake and facilitate client transitions.
• Lead and supervise Transition Specialists.

• Prepare reports on admissions, census, and outcomes.
