Job summary

ACMH, INC. is seeking a Director of Intake & Transition Services in Manhattan. The role coordinates all agency intake and promotes client transitions to more independent living, reporting to the Senior Director for Residential Programs. Standard schedule is Monday to Friday, 9:00am–5:00pm.

The incumbent will supervise Transition Specialists, liaise with hospitals and clinics, and contribute to service quality improvement across ACMH programs.

Qualifications

• Requires advanced degree in psychology or related field or a BA with substantial relevant experience.
• Experience in coordinating intake, transitions to independent living, and liaising with program staff is advantageous.

Responsibilities

• Reviews applications and screenings, presents referrals to agency teams, and supports admissions.
• Assists referral sources when applicants do not meet admission criteria.
• Liaises with all agency programs regarding admissions and internal transfers.
• Provides consultation to staff on transition opportunities to more independent settings.
• Identifies eligible applicants for CORE & Respite programs and other ACMH services.
• Maintains liaison with hospitals, clinics, rehab programs, shelters and homeless drop-in centers.
• Maintains internal vacancy memo and census reports; reports to City and State agencies.
• Manages post-discharge survey and outcomes management system.
• Facilitates quarterly transition meetings and centralized intake meetings.
• Supervises Transition Specialists and recommends service quality improvements.

Function: Responsible for coordinating all agency intake and promoting client transitions to more independent living.

Reports to: Senior Director for Residential Programs

Location: Manhattan

Schedule: Mondays through Fridays (9:00am-5:00pm)

Tasks

• Reviews applications, sets up screenings, presents referrals to agency teams and recommends admissions in accordance with agency policy.
• Aids and assists referral sources to other housing providers if applicants do not meet ACMH admission criteria.
• Serves as the liaison to all agency programs in regards to admissions and internal transfers.
• Provides consultation to program staff on opportunities for transition to more independent settings.
• Assists with identifying eligible applicants for CORE & Respite programs as well as other non-residential ACMH services.
• Interprets the agency’s programs and maintains liaison with hospitals, outpatient clinics and rehabilitation programs, shelters and drop-in centers for the homeless.
• Maintains and disseminates internal vacancy memo to report on status of tours, admissions, internal transfers, discharges and census.
• Maintains waitlist of applicants for programs.
• Maintains client census reports and reports regularly to City and State agencies on admissions, discharges, vacant unit status, and leaves of absence.
• Manages post-discharge survey and outcomes management system.
• Facilitates quarterly transition meetings for all agency programs.
• Facilitates Centralized Intake meetings.
• Supervises Transition Specialists.
• Recommends measures to evaluate and improve the quality of program services.
• Attends in-service training as requested by supervisory personnel.
• Serves as “Qualified Mental Health Specialist”.
• Duties as assigned by supervisor.

Requirements

M.A. in the field of psychology or an analogous field or B.A. with significant experience in the field.

Salary: $85,000 - $90,000 commensurate with experience, plus generous benefits.
