Responsibilities

• Lead all Case Management department operations within the hospital
• Oversee utilization review, discharge planning, and care coordination activities
• Manage and mentor a team of Case Managers
• Recruit, onboard, and develop new Case Management staff
• Support improvements in patient experience and discharge planning outcomes
• Collaborate with physicians, nursing leaders, therapists, and interdisciplinary teams
• Monitor departmental performance metrics and implement process improvements
• Ensure compliance with regulatory and accreditation requirements
• Assist with utilization management initiatives and resource optimization
• Carry a patient caseload as needed while supporting department operations
