Job summary

LeoForce is seeking a Director of Case Management to lead an 11-person team of RNs, LPNs, MSWs, LSWs and other clinicians at our inpatient rehab facility serving the Yuma, AZ region. The role emphasizes safe discharge planning, strong interdisciplinary collaboration, and active participation in utilization review.

You'll partner with hospital leadership, drive departmental strategy, and have the opportunity to pursue CCM/ACM certification within the first year while contributing to a major

Qualifications

• Experience leading a case management department in an acute care or rehab setting.
• Proven ability to coordinate discharge planning across disciplines.
• Knowledge of Utilization Review processes and patient flow.
• Strong communication with nursing, therapy, and hospital leadership.

Responsibilities

• Provide leadership and oversight for the Case Management department, including 11 direct reports.
• Guide safe, timely discharge planning across the hospital.
• Support Utilization Review processes with the case management team.
• Monitor caseloads and step in when census is low or staffing is short.
• Maintain knowledge of patient assignments and support the team as needed.
• Foster interdisciplinary communication across nursing, therapy, and leadership.
• Partner with RDCM on training, orientation, and education initiatives.
• Participate in hospital leadership meetings and contribute to strategy.

Job Details

• Provide leadership and oversight for the Case Management department, including 11 direct reports (RNs, LPN, MSWs, LSW, and other allied health professionals)
• Guide safe, timely, and effective discharge planning across the hospital
• Support Utilization Review processes in coordination with the case management team
• Monitor and manage case manager caseloads (typical caseload of 13 patients per CM); step in to carry a patient assignment when census is low or staffing is short
• Maintain working knowledge of case managers' patient assignments and actively support the team as needed
• Foster strong interdisciplinary communication across nursing, therapy, and hospital leadership
• Partner with the RDCM on training, orientation, and ongoing education initiatives
• Participate in hospital leadership meetings and contribute to department strategy
