Job summary

Jobot is seeking a dynamic leader to guide an 11-person Case Management team at Yuma’s inpatient rehab hospital. The role focuses on utilization review, care coordination, appropriate discharges, and census management to support safe, high-quality outcomes.

You will oversee interdisciplinary plans of care, collaborate with leadership, and drive performance against quality metrics in a 61-bed facility undergoing a private-room expansion through 2027.

Qualifications

• Qualified to independently complete an assessment within the scope of practice (RN preferred; SW/OT/PT/ST or Rehab Counseling eligible).
• Active license required if licensure exists for that discipline in Arizona.
• 3 years of hospital-based Case Management experience including Utilization Review and Discharge Planning.

Job Details & Responsibilities

• Own day-to-day operations and people leadership for the Case Management department (11 direct reports)
• Oversee utilization review, care coordination, interdisciplinary plans of care, and discharge planning
• Drive census management, patient-care outcomes, and key care indicators
• Know assigned case managers’ patients; pitch in and cover when census is low or the team is short-staffed
• Guide safe, timely discharges and effective handoffs across nursing, therapy, physicians, and post-acute partners
• Coach and develop a mixed-discipline team (3 RN, 1 LPN, 3 MSW, 1 LSW, 1 BA Psychology, 1 BS HCA)
• Partner with hospital leadership and the Regional Director of Case Management on quality, compliance, and staffing
• Model strong interdisciplinary communication and proven leadership on the floor

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of the discipline (RN preferred; SW, OT, PT, ST, or Rehabilitation Counseling also eligible)
• Active license required if licensure exists for that discipline in Arizona
• Nursing path: BSN with active RN license (preferred target)
• Other eligible disciplines: bachelor’s degree required; graduate degree preferred
• 3 years of hospital-based Case Management experience including Utilization Review and Discharge Planning
• CCM or ACM required, or obtained within the first year
• Proven effective leadership skills
• Effective interdisciplinary communication skills
