Job summary

Leoforce is seeking a Director of Case Management to oversee day-to-day operations of the Case Management department in our inpatient rehabilitation setting. You will lead Case Managers and support staff and coordinate discharge planning with physicians and care teams.

This role reports to the CEO, mentors a team of 11 direct reports, drives quality and patient outcomes, and partners with insurers and healthcare networks.

Qualifications

• Director-level experience in case management or healthcare operations.

Responsibilities

• Assume responsibility for day-to-day operations of the Case Management department
• Lead and manage Case Managers and Case Management support staff
• Oversee interdisciplinary plans of care and discharge planning processes
• Ensure effective patient transitions and care coordination
• Monitor census management, patient outcomes, and key quality indicators
• Serve as an advocate for patients and families to ensure appropriate services and support
• Provide coaching, mentoring, and professional development to Case Management staff
• Interpret and implement regulations, policies, operational procedures, and organizational objectives
• Review departmental operations and maintain quality standards consistent with organizational expectations
• Collaborate with physicians, therapists, nursing staff, and interdisciplinary teams
• Build relationships with insurance companies, self-insured employers, case management firms, and healthcare networks
• Support utilization review activities and patient progression initiatives
• Identify opportunities for operational improvement and enhanced patient outcomes
