This full-time, 8:00–4:00, Monday–Friday role emphasizes effective discharge planning and collaboration with physicians and caregivers.

You will review THRIL scores daily, identify high-risk patients, and initiate transition evaluations within 24 hours of identification.

Qualifikationen

• RN is required.
• Bachelor's Degree in Nursing is required.
• ACM/ CCM or other ANCC certifications preferred or upon hire preferred.

Aufgaben

• Ensure patients are transitioned to appropriate levels of care in a timely and effective manner.
• Review THRIL scores daily for all assigned patients.
• Identify high risk patients and promote discussion for PCP identification.
• Complete Transition Evaluations within 24 hours of identification and begin discharge planning.
• Interview and assess patients and caregivers as part of the transition evaluation.
• Discuss funding of post-transition care with patients and caregivers.
