Responsibilities

  • Provide clinical direction and support to APPs across clinical services programs.
  • Develop and implement clinical protocols, policies, and workflows.
  • Serve as a resource for complex, urgent, and emergent clinical situations.
  • Lead, manage, and mentor a team of APPs.
  • Oversee recruitment, onboarding, training, and ongoing professional development.
  • Conduct performance evaluations, assess competencies, and implement corrective action plans as needed.
  • Conduct chart reviews to identify documentation gaps and opportunities for improvement.
  • Lead initiatives to improve documentation accuracy, coding integrity, and quality gap closure.
  • Develop and implement risk adjustment improvement strategies and remediation plans.
  • Provide education and training on clinical documentation and best practices.
  • Establish strategic goals aligned with organizational priorities.
  • Monitor program performance and implement improvement strategies.
  • Coordinate with cross-functional teams to ensure appropriate resources, equipment, and operational support.
  • Build and maintain relationships with internal teams, providers, and external stakeholders.
  • Collaborate with healthcare providers to ensure coordinated, high-quality care.
  • Participate in performance and operational meetings with leadership and partners.
  • Support management of program performance in alignment with scope of work (SOW) and regulatory requirements.
  • Monitor performance trends and contribute to action plans addressing gaps or underperformance.
  • Prepare and present clinical, quality, and operational outcomes to internal stakeholders.
  • Ensure compliance with all HIPAA regulations and protection of PHI.
  • Maintain and enhance clinical knowledge through continuing education and professional development.
  • Participate in quality assurance and process improvement initiatives.

POSITION PURPOSE

Adobe Population Health is seeking a highly motivated and experienced Nurse Practitioner Manager to oversee and manage our clinical services program. The ideal candidate will be responsible for leading a team of advanced practice providers (APPs) in our population health and Adobe Provider Group (APG) populations managing the following areas:

  • Advanced practice provider leadership: Oversee and lead teams of Advanced Practice Providers (APPs) across population health and Adobe Provider Group (APG) service lines, ensuring high-quality, coordinated care delivery.
  • Clinical program development: Design and implement evidence-based protocols for chronic, preventive, palliative, and acute care, tailored to the needs of diverse patient populations.
  • Data-driven performance improvement: Partner with Quality and Analytics teams to evaluate program outcomes, leveraging data to identify opportunities to enhance care delivery, patient outcomes, and operational efficiency.
  • Regulatory compliance and quality oversight: Ensure adherence to Centers for Medicare & Medicaid Services (CMS), as well as federal and state regulatory requirements, while leading continuous quality improvement initiatives.
  • Eventa integration and oversight: 25% of position dedicated to clinical oversight and process optimization for Eventa to expand and strengthen service line capabilities for members with complex respiratory therapy needs.
DUTIES & RESPONSIBILITIES
Clinical Leadership and Oversight

SKILLS & QUALIFICATIONS
  • Five (5) plus years of clinical experience
