Legacy Community Health in Houston seeks a Director – Medication Adherence & Access to lead programs ensuring patients receive necessary medications and education. You’ll oversee adherence initiatives and collaborate with pharmacy, operations, and clinical teams to reduce barriers to care.

The role involves developing strategies to improve adherence, coordinating with leaders and providers, and mentoring pharmacists and staff to deliver patient-centered services across Legacy’s network.

Qualifications

• Graduate of an accredited college/university of Pharmacy (Pharm.D or B.S. with equivalent experience).
• Current Texas Pharmacist licensure in good standing (or eligible).
• Certification in immunization delivery, smoking cessation, and naloxone.
• Post-graduate pharmacy residency required.

Responsibilities

• Lead programs related to medication adherence, medication access, prescription renewals, prior authorization assistance, and medication assistance programs.
• Oversee activities to minimize delays in patient medication needs and ensure timely delivery of services.
• Coordinate standardization of activities and communicate with leaders to support program success.
• Evaluate innovative approaches to improve adherence rates and patient outcomes across Legacy clinics.
