Job summary

CommonSpirit Health at Home seeks a Director of Clinical Services to lead the planning, direction, and evaluation of clinical services across the facility. You will supervise teams, ensure quality patient care, and maintain compliant clinical records.

You will champion mission and values, ensure regulatory adherence, and drive performance through coaching, mentorship, and process improvements across EMR workflows and staffing.

Qualifications

• Minimum 3 years of management experience in home health or hospice.
• Graduate RN with active license (BSN preferred) or licensed PT/OT in good standing.
• Current CPR or BLS certification.
• Two years of home health or hospice experience; hospital nursing as a plus.

Responsibilities

• Ethical Conduct & Compliance: Uphold the Corporate Compliance Program.
• Operational Support: Provide prompt and professional assistance to staff.
• Quality Care Oversight: Direct the accurate, complete, and timely provision of quality patient care and services, ensuring individualized patient plans of care are appropriate and in compliance with all regulatory, operational, and financial requirements.
• Clinical Record Compliance: Ensure accuracy, completeness, and timeliness of clinical records.
• EMR Workflow Completion: Guarantee that all HCHB clinical workflow for both DCS and MCS is completed and current by end of each business day.
• Outcome Monitoring & Improvement: Monitor clinical outcome results and implement improvement plans.
• Clinical Performance Management: Monitor and evaluate clinical performance measures and implement action plans.
• Clinical Documentation Expertise: Ensure ongoing development of clinical staff knowledge and correct completion of orders and documentation tools.
• Information Dissemination: Ensure accurate communication of regulatory, operational, and financial information.
• Performance Excellence: Establish and manage expectations, coach, mentor, and provide feedback.
• Staffing & Productivity: Maintain staffing levels and monitor productivity.
• Associate Development: Provide education and support career goals and morale.
• Branch Involvement: Participate in strategic planning, budget development, and financial analysis.
• Administrator of Record: May serve as Administrator of Record with State Regulatory agency.
