Qualifications

• Associates or Bachelors in Nursing preferred; RN/LPN licensure may be considered depending on state.
• Experience leading health and wellness teams in assisted living or memory care environments.
• Strong communication, organizational, and problem-solving abilities.

Responsibilities

• Perform comprehensive health assessments for new residents upon admission.
• Monitor and review health status through care conferences and assessments.
• Ensure staff compliance with care plans, medication protocols, and resident needs.
