Qualifications

• Bachelor’s degree in Healthcare Administration, Business, or a related field.
• 5+ years of progressive leadership experience in healthcare or patient services operations.
• Experience managing teams in a complex, matrixed healthcare organization.
• Experience delivering high-touch, patient-centered services for complex or high-acuity populations.

Responsibilities

• Oversee day-to-day operational performance of the destination care program and referral workflows.
• Ensure concierge-caliber service from initial contact through post-discharge; conduct patient/family rounding to identify gaps.
• Lead process improvements and workflow optimization to enhance efficiency, service quality, and patient satisfaction.
• Support growth of the destination care program by expanding referrals and strengthening market position.
