Job summary

Northwell Health in New York is seeking an experienced nursing leader to direct and evaluate all activities of the case management program across the continuum of care. You will develop processes to monitor utilization review, LOS, and resource reduction strategies, and provide data analytics to drive performance improvements.

The role requires a Master’s in Nursing, active New York State RN licensure, and 8–12 years of relevant experience with at least 7 years in leadership.

Qualifications

• Masters or higher in Nursing or related field.
• New York State RN license required with applicable certifications.
• 8–12 years of relevant experience and 7+ years leadership/management experience.

Responsibilities

• Leads a functional unit by developing goals and a short-term strategic vision for areas of responsibility.
• Directs case management across the continuum of care and collaborates with hospital leadership.
• Analyzes patient care standards and plans resource use to improve quality and efficiency.
• Interprets and implements policies and procedures to support quality patient care.
• Mentors staff, engages teams, and evaluates patient satisfaction and clinical outcomes.
