Job summary

Mosaic Health is seeking a Director to lead Burden of Illness initiatives across physician practices. You will translate BOI strategy into actionable programs, collaborating with Risk Adjustment, Population Health, Finance, and Analytics teams to improve documentation and coding accuracy.

The role requires in-depth CMS-HCC knowledge, strong leadership, and the ability to drive measurable performance improvements within value-based care arrangements.

Qualifications

• Bachelor's degree in Healthcare Administration, Business Administration, Health Information Management, Public Health (or equivalent).
• CRC or CPC certification strongly preferred.
• 7+ years in Medicare Risk Adjustment, Burden of Illness, healthcare analytics, or related operations.
• 3–5 years of leadership experience.
• Experience with CMS-HCC methodologies and reporting.

Responsibilities

• Provide leadership for Burden of Illness initiatives and maintain current knowledge of CMS-HCC rules.
• Monitor BOI indicators and identify opportunities for provider intervention.
• Establish BOI priorities for physician practices based on data.
• Develop scalable workflows integrating BOI activities into routine operations.
• Coordinate BOI initiatives with Risk Bearing Entity partnerships and leadership.
• Ensure BOI initiatives are tracked, prioritized, and executed across the organization.
