Job summary

Seven Hills Foundation is seeking an Activities Specialist to empower individuals with disabilities by delivering engaging personal care support and skill-building activities in a supportive setting in Harvard, MA.

You will develop and lead individual and group activities, assist with dining, toileting, transfers, and transportation, and document progress in ISPs to help participants achieve independence and meaningful community participation.

Qualifications

• High School diploma and one year related experience.
• Valid driver’s license required (Class C or D1).
• Experience in personal care or supportive services preferred.

Responsibilities

• Support personal care needs: assist individuals with dining, toileting, and transfers to maintain comfort and dignity.
• Develop and lead engaging individual and group activities to promote skill development and independence.
• Collaborate with clinical team on behavior plans tailored to each person.
• Facilitate transportation to activities and appointments as needed.
• Evaluate progress, complete progress documentation, and ensure ISP compliance.
