Job summary

Seven Hills Foundation is seeking an Activities Specialist to empower individuals with disabilities, supporting daily living tasks and leading engaging activities in a team-oriented environment.

You will implement programs, help with transportation, and document progress to meet ISP requirements, while collaborating with clinicians to tailor behavior plans for each participant.

Qualifications

• High School diploma required; one year related experience preferred.
• Ability to support personal care needs and assist with dining, toileting, and transfers.
• Experience developing and leading individual and group activities.
• Collaborative work with clinical team to implement behavior plans.

Responsibilities

• Support Personal Care Needs: Assist individuals with dining, toileting, and transfers, ensuring their comfort and dignity.
• Implement Engaging Programs: Develop and lead individual and group activities that promote skill development, independence, and community integration.
• Collaborate on Behavior Plans: Work with the clinical team to create and implement behavior management strategies tailored to each individual.
• Facilitate Transportation: Assist with transporting participants to various activities and appointments as needed.
• Evaluate and Document Progress: Conduct satisfaction surveys, document participant progress, and ensure compliance with ISPs.
