Job summary

Journey Found, Inc. is seeking a Direct Support Specialist to assist individuals with daily living in our Connecticut group homes. This full-time role focuses on personal care, transportation, medication administration, and program data documentation.

You will collaborate with nurses and supervisors, implement person-centered routines, and help individuals participate in community activities while maintaining a safe and supportive environment. A valid driver’s license is required.

Qualifications

• High School Diploma or equivalent
• 3 years driving experience with valid license
• Behavioral management experience or formal training
• Ability to communicate effectively, both orally and in writing
• PMT, CPR, FA and complete DDS Medication Administration Program (MAP) required

Responsibilities

• Assist individuals with activities of daily living (dressing, cooking, cleaning, bathing, toileting).
• Transport individuals to appointments and activities.
• Organize activities and community outings; document program data.
• Administer medications per MAP and coordinate with nurses/clinical staff.
• Deescalate situations and support positive behavior management.
• Communicate professionally with team members and supervisors.
