Job summary

Responsibilities

• Review and implement ISP and BSP.
• Provide support in a respectful, person-centered manner.
• Monitor physical and emotional health of individuals.
• Follow agency policies and procedures.
• Submit required documentation by deadlines.
• Maintain open, positive communication with supervisors and stakeholders.
• Attend trainings as scheduled and follow staffing rotations.
• Must be able to transport individuals during scheduled hours.
