Job summary

Evergreen Life Services is seeking compassionate Direct Support Professionals to provide essential care in our residential and day programs. You will help individuals with medication, daily activities, budgeting, and transportation while maintaining a safe, nurturing environment aligned with each person’s plan of care.

Qualifications

• Must be at least 21 years old due to auto liability coverage.
• Ability to read, write, and communicate in English; capable of record-keeping and reporting.
• Meet state agency requirements including good standing with abuse/neglect registry.
• Learn basic computer skills (turn on/off, log in/out, input data, timesheets).

Responsibilities

• Maintain a healthy, safe, and therapeutic environment as per individual plans of care.
• Assist individuals with medication and treatment plans as needed.
• Assist with home care tasks and reporting maintenance needs.
• Support money management activities (budgeting, banking, shopping).
• Help with daily life activities—meals, reading, telephone use, community services, housekeeping, self-care.
• Purchase food and supplies, prepare meals, and maintain home/grounds.
• Complete required documentation per state agencies and Evergreen procedures.
• Implement behavioral programs and assist with goals outlined in care plans.
• Identify triggers and defuse or redirect to ensure safety.
• Provide transportation as needed in company or personal vehicle.
