Job summary

You’ll provide supports in residential or day center settings and work within a collaborative care team while modeling respect and dignity. Responsibilities include implementing individual plans, assisting with personal hygiene and daily activities, documenting care, ensuring safety, and communicating with supervisors.

Qualifications

• Must pass prehire requirements including criminal background check and drug screen.
• Must maintain a valid driver’s license and auto liability insurance.
• Prior Direct Support Professional experience preferred.
• Maintain confidentiality and HIPAA awareness in all duties.

Responsibilities

• Remove barriers and provide supports in residential or day center settings.
• Assist people to lead self-directed lives and participate in community activities.
• Support plans and goals; assist with personal care tasks and daily activities.
• Communicate with supervisors and document care promptly.
• Ensure safety and maintain a clean, respectful environment.
