Job summary

Responsibilities

• Assist individuals with Activities of Daily Living (ADLs) such as grooming, cooking, cleaning and personal hygiene.
• Support participation in community activities and recreation.
• Accompany individuals to appointments, shopping, or social outings.
• Encourage skill-building for community engagement, including budgeting and transportation use.
• Implement and document individualized support plans and ISP outcomes.
• Teach and reinforce independent living skills like cooking and personal safety.
• Support communication and self-advocacy efforts.
• Report incidents or changes in condition to the supervisor promptly.
