Job summary

Qualifications

Responsibilities

• Assist individuals with Activities of Daily Living (ADLs) such as grooming, cooking, cleaning and personal hygiene.
• Support individuals in accessing and participating in community activities and recreation.
• Accompany individuals to appointments, shopping, or social outings as needed.
• Encourage skill-building for community engagement (money management, transportation use, social interaction).
• Implement and document individualized support plans and ISP outcomes.
