Qualifications

• CPR/First Aid certification required
• Authorization to work in the U.S.
• Clear background check
• High School Diploma or equivalent
• Minimum 1 year of healthcare experience

Responsibilities

• Assist individuals with Activities of Daily Living (ADLs) such as grooming, cooking, cleaning and personal hygiene.
• Support individuals in accessing and participating in community activities and recreation.
• Accompany individuals to appointments, shopping, or social outings as needed.
• Encourage skill-building for community engagement (money management, transportation, social interaction).
• Implement and document individualized support plans and ISP outcomes.
• Teach and reinforce skills promoting independence (cooking, budgeting, personal safety).
• Support communication and self-advocacy efforts.
• Report incidents, changes in condition, or concerns to the supervisor promptly
