Job summary

Qualifications

• High School Diploma or equivalent preferred.
• One year of care experience with developmental disabilities or seniors is acceptable.

Responsibilities

• Assist with personal care, grooming, feeding, toileting, and meal prep.
• Support mobility, community access, and learning new skills.
• Coordinate medical appointments and transportation needs as directed.
• Provide companionship and supervision during community activities.
• Maintain daily logs including notes and medication records.
• Report changes in health or behavior to supervisor.
• Ensure health and safety during shifts and assist with medications as needed.
• Help maintain the consumer's home and equipment; report repairs.
• Follow ISP and daily task lists as assigned by DSP 2/3 and Area Coordinator.
