Qualifications

• High School Diploma or equivalent is preferred.
• One year of experience providing care to persons with developmental disabilities or in the human service field is acceptable.

Responsibilities

• Assist or perform for the consumer in personal care tasks (grooming, feeding, toileting, meal preparation).
• Provide basic housekeeping tasks, laundry, grocery shopping as assigned.
• Support mobility, community access, and learning new skills for the consumer.
• Assist with medical appointments and communication, schedule travel and activities as assigned.
• Provide companionship, guidance, and supervision during community activities.
• Record daily notes in consumer logs and medication log.
• Report any changes in health or condition to supervisor immediately.
• Maintain the consumer's health and safety during shifts and monitor medication administration.
• Assist in home upkeep and equipment maintenance; report damages or needed repairs.
