JOB TITLE: DIRECT SUPPORT PROFESSIONAL 1 - NON-EXEMPT

Position Description:

Support adults with developmental disabilities to accomplish daily living activities at the person’s home and/or in the local community. Help the individual to be a known and valued member of his/her local community. The attendant is directly responsible for personal care and support to meet the goals and objectives of the individual that they serve. The Direct Support Professional 1 (DSP 1) shall be aware that the consumers may terminate individual service. FUTURE TRANSITIONS, INC., is an At-Will employer, meaning that the employer or employee with or without cause, adhering to the law governing employee/employer relationships, may terminate employment at any time. The DSP will understand that FUTURE TRANSITIONS, INC. has an obligation to the clients we serve and, due to their vulnerability, the DSP’s employment is contingent upon the completion of a criminal Live Scan background check (This screening will only be initiated after a conditional offer of employment has been made), a DMV report for the past 7 years and a TB test or official proof of a negative TB test result completed within 12 months prior to the date of hire. Must be at least 18 years of age.

1. Assist or perform for the consumer(s) in personal care, such as grooming, feeding, toileting, and meal preparation. Assist or perform for consumer(s) basic housekeeping tasks, laundry and grocery shopping, and other personal care services as assigned by DSP 2.

2. Support and guide the consumer in the choices they make regarding mobility, community access, and learning new skills.

3. Assist or facilitate: medical appointments, communication, equipment repair, accessing recreational activities, prescribed exercises, and schedule generic travel arrangements and all activities as assigned by the DSP 2.

4. Provide companionship, guidance, and supervision during Community activities.

5. Record daily in the consumer logs. This includes, but is not limited to, the following: SLS Consumer Daily Notes, Medication Log.

6.Report immediately any changes in the consumer’s mental, physical, or emotional condition to the supervisor.

7. Ensure consumer’s health and safety during assigned hours of responsibility. Ensure that the consumer takes self-administered medication.

8. Assist in the overall maintenance of the consumer’s home and general upkeep of adaptive equipment as assigned. Report any damages to the property or needed repairs for safety purposes immediately to the Direct Support Professional 2 (DSP 2). Abide by all home rules, property agreements, and equipment warranties.

9. Perform all duties as specified by the consumer, the Direct Support Professional 2, and 3 (DSP 2, and 3) and the Area Coordinator as detailed in the ISP and the Daily Task List.

Qualifications and Requirements

Education and Experience:

High School Diploma or equivalent is preferred.

One year of experience providing care to persons with developmental disabilities. Experience working in the human service field is also acceptable.

Transportation:

a. Valid California driver's license.

b. Current vehicle registration and use of a vehicle in good running condition during shift hours.

c. Valid automobile insurance for the vehicle you are driving for work and all passengers, with the employee name listed on the policy (new verification is required at the expiration of the policy).

Other:

• Current CPR and First Aid Certification for adults and children.
• Cell phone with data plan and app capabilities.

TB Testing Requirement:

Employment is contingent upon clearing a negative TB test. Testing is fully covered if completed at the employer's designated facility. Candidates who use an outside provider will be reimbursed up to the cost of our standard provider fee upon submission of a receipt. Alternatively, official proof of a negative TB test completed within 12 months prior to the date of hire is also accepted.

Pay Range: Based on experience - $19.50 - $21.50 hourly
