Job summary

Achieve is seeking a Direct Support Professional for self-direction in Madison County, NY. This per diem role adapts to individuals' needs, helping them build daily living and community skills while promoting independence and quality of life.

You will provide one-on-one support, assist with transportation and documentation, and collaborate with the Self-Directed participant to meet personal goals. Must have a High School Diploma or GED and a valid NYS driver's license.

Qualifications

• High School Diploma or GED is required.
• Valid New York State Driver's License is required.

Responsibilities

• Provide individualized support in accordance with each person’s service plan, preferences, needs, and personal goals
• Promote independence and skill development by assisting individuals with daily living, self-help, socialization, and adaptive skills
• Support personal and community-based goals that encourage independence, productivity, meaningful participation, and community integration
• Provide safe transportation and First Aid/CPR support as requested by the Self-Directed participant
• Complete required documentation accurately and on time, including EVV requirements, monthly summaries, mileage records, reimbursement submissions, and other required reports
• Maintain confidentiality, professionalism, and compliance with agency policies and procedures while attending required meetings and trainings

This role helps individuals build and maintain daily living, social, and adaptive skills while working toward personal goals, greater independence, community integration, and an enhanced quality of life.
