Job summary

ACHIEVE NY is seeking a Direct Support Professional for a self-directed setting in Madison County, NY. This per diem role offers starting wage of $18 per hour and up, with schedule based on individuals' needs and program requirements.

You will support people with developmental disabilities to build daily living and social skills, promote independence, and participate in community activities while ensuring safety and documentation compliance.

Qualifications

• Required First Aid/CPR knowledge or certification.
• Experience supporting individuals in self-directed settings is a plus.
• Valid NY driver’s license is required.

Responsibilities

• Provide individualized support in accordance with each person’s service plan and goals.
• Promote independence and daily living skill development.
• Support personal and community-based goals for independence and participation.
• Provide safe transportation and First Aid/CPR as requested.
• Complete required documentation and EVV compliance on time.

Responsibilities

The Direct Support Professional provides individualized support to people with developmental disabilities in a self-directed setting. This role helps individuals build and maintain daily living, social, and adaptive skills while working toward personal goals, greater independence, community integration, and an enhanced quality of life.

• Provide individualized support in accordance with each person’s service plan, preferences, needs, and personal goals
• Promote independence and skill development by assisting individuals with daily living, self-help, socialization, and adaptive skills
• Support personal and community-based goals that encourage independence, productivity, meaningful participation, and community integration
• Provide safe transportation and First Aid/CPR support as requested by the Self-Directed participant
• Complete required documentation accurately and on time, including EVV requirements, monthly summaries, mileage records, reimbursement submissions, and other required reports
• Maintain confidentiality, professionalism, and compliance with agency policies and procedures while attending required meetings and trainings
