ACHIEVE NY is seeking a Direct Support Professional - Residential in Madison, NY 13402. This full-time role provides direct care and supervision in a residential setting, helping residents develop independence in daily living activities while prioritizing health and safety.

The job involves assisting with ADLs, supporting independent living skills, money management, and transportation to appointments and activities, along with documentation and teamwork.

Qualifications

• High School Diploma or GED required.
• Experience in human services preferred.
• Valid NY State driver's license with acceptable driving record.
• Ability to lift up to 50 pounds and assist with transfers.

Responsibilities

• Assist residents with activities of daily living including bathing, grooming, hygiene, and toileting
• Support development of independent living skills such as cooking, housekeeping, laundry, and shopping
• Support money management skills including budgeting and making appropriate purchases
• Administer medications and complete required documentation in accordance with agency policy
• Transport and accompany residents to medical appointments, recreational activities, and community outings
• Assist with daily household operations and maintain a clean, organized living environment
• Participate in assessments, service planning, and team meetings
• Communicate resident health, safety, and behavioral concerns to supervisors
• Ensure compliance with OPWDD regulations, including Part 633 and Part 624
• Attend required training and meetings and stay up to date on communications
• Maintain reliable attendance and support staffing needs as required
• Perform any additional duties as assigned
