Job summary

Achieve is seeking a Direct Support Professional – Overnight in Madison, NY. This full-time role covers 2nd and 3rd shift, weekends and holidays as needed, providing direct care and supervision to residents in a residential setting.

You’ll help residents with ADLs, support independent living skills, manage medications per policy, transport to appointments, and participate in team planning while maintaining safety and compliance.

Qualifications

• High School Diploma or GED is required.
• Valid NY State driver's license with acceptable driving record.
• Ability to lift and assist with up to 50 pounds.

Responsibilities

• Assist residents with activities of daily living including bathing, grooming, hygiene, and toileting.
• Support development of independent living skills such as cooking, housekeeping, laundry, and shopping.
• Support money management skills including budgeting and making appropriate purchases.
• Administer medications and complete required documentation in accordance with agency policy.
• Transport and accompany residents to medical appointments, recreational activities, and community outings.
• Assist with daily household operations and maintain a clean, organized living environment.
• Participate in assessments, service planning, and team meetings.
• Communicate resident health, safety, and behavioral concerns to supervisors.
• Ensure compliance with OPWDD regulations, including Part 633 and Part 624.
• Attend required training and meetings and stay up to date on communications.
• Maintain reliable attendance and support staffing needs as required.
• Perform any additional duties as assigned.

Direct Support Professional Key Responsibilities

The Direct Support Professional – Residential provides direct care, supervision, and support to individuals in a residential setting. Requirements

• High School Diploma or GED required
• Experience working in human services or with individuals with functional differences preferred
• Valid NY State driver’s license with a driving record acceptable to the agency’s insurance carrier
• Ability to lift and assist with up to 50 pounds, including transfers and physical support
