Job summary

Achieve seeks a Direct Support Professional – Residential Summary to support individuals in Greene, Oxford, and Norwich NY. This full-time role focuses on helping residents build independence and life skills in a team-driven environment.

You will provide training, assist with daily living activities, supervise in-home and community activities, and contribute to accurate documentation and care plans. Competitive pay and benefits offered.

Qualifications

  • High School diploma or GED required.
  • Must possess a valid NY State driver’s license acceptable to the agency’s insurance carrier.
  • Must be physically able to perform required certifications and lift up to 50 pounds.

Responsibilities

  • Provide training and assistance to individuals to develop independent living skills (e.g., cooking, housekeeping, laundry, budgeting).
  • Provide training in personal care skills (showering, toothbrushing, toileting, bathing).
  • Provide supervision to residents at the residence and in the community as appropriate.
  • Complete documentation and records (T-logs, habilitation plans, progress reports) in a timely manner.
  • Administer resident medications and logging according to procedure.
  • Assist residents with daily tasks and accompany them on outings and appointments.

Direct Support Professional – Residential Summary


Location: Positions in Greene, Oxford, and Norwich NY


Schedule: Full-time


Salary: $20.00 - $21.75

Direct Support Professional / Residence Aide’s make a meaningful impact by helping individuals build independence and life skills in a supportive, team-focused environment. DUTIES AND RESPONSIBILITIES:


  • Provide training and assistance to individuals in order to help them develop and/or improve independent living skills such as cooking, housekeeping, laundry skills, grocery shopping, etc.

  • Provide assistance and training to individuals to assist them in developing and/or improving personal care skills; i.e. showering, toothbrushing, toileting, bathing, etc.

  • Provide supervision to individuals at the residence and in the community as appropriate.

  • Provide training in money management such as budgeting, banking, and making appropriate purchases.

  • Complete all documentation and records accurately and within appropriate time frames; T-logs, Habilitation plans, Progress reports and any other assigned documentation.

  • While clocked in, employees are required to read agency e-mails daily through the agency utilized email systems If an employee is unable to access the e-mail systems, they are to contact their supervisor and the Information Technology Help Desk for assistance.

  • Due to the critical nature of our Agency’s work, and required 24-hour coverage, the Residential Program always remains open it is imperative and expected that all employees (Residential & Day Habilitation) make every effort to report to work in the event of inclement weather, including, but not limited to, snow, ice, freezing rain, or flood. Employees should also make every effort to remain at work in the event inclement weather occurs during their work schedule until relieved of duty by the following shift.

  • This position will at times require you to travel or transfer between one work location/site and another per a supervisors request. Approved transportation with personal vehicles is reimbursable at the per mile rate set by IRS guidelines. Mileage, from a regularly assigned place of work to a temporarily assigned place of work can submitted for reimbursement.

  • Follow assigned work schedule and placement. Your specific work schedule and work site placement will be determined by program needs and will be discussed with you by your supervisor, PMOC or AOC and is subject to change.

  • Administer resident medications and logging according to procedure.

  • Accompany residents to grocery stores and other stores for on-site training in purchasing.

  • Attend case conferences, screenings, discharge conferences and meetings as requested.

  • Perform/participate in assessment and service plan development and implementation.

  • Maintain a positive relationship with parents, participants and other staff.

  • Transport residents and supervise on appointments, recreational and/or other activities as needed. This includes but is not limited to following all policies and procedures specific to recreational/community activities.

  • Keep supervisor informed of any residents’ health and safety issues.

  • Assist residents with or, if necessary, completing daily or routine domestic tasks.

  • Maintain the residence by cleaning as instructed by the supervisor any part of the house and/or its contents.

  • Assist with filing, record keeping and maintaining an orderly office.

  • Adhere to Part 633 and Part 624 Regulations related to Consumer Rights, Abuse and Reporting.

  • Adhere to Incident and Accident reporting per policies and procedures.

  • Advocate for the rights of the developmentally disabled.

  • Attend scheduled house meetings.


Requirements


  • At Minimum High School diploma or GED

  • Possess a valid NY State driver’s license acceptable to the agency’s insurance carrier.

  • Must physically be able to perform required certifications and lift fifty (50) pounds experience

  • Some knowledge of or experience working with developmentally disabled preferred.
